- To schedule a consultation with Dr. Makeba; please fill out the form below or call 877-491-7837 & choose option 1, once you have received your consultation time from a representative you will be asked to return to this page to pay for your consultation to complete the scheduling process.
- When choosing a time for your consultation please choose a time that you will have 30 min on either time of the start time to begin your consultation example: If you have a 1:00pm consultation we ask that you be available to begin the consult from 12:30pm to 1:30pm.
